In order to quickly identify children at risk for vaccine-preventable diseases in the event of an outbreak, licensed child care centres must submit a current enrollment list annually or as requested by Public Health.
The following information must be included in each list:
- Name of child care centre
- Child’s first and last name
- Date of birth
- Parent/Guardian’s phone number
- If the child attends a before-school program, after-school program or both
Licensed child care centres that do not submit updated enrollment lists will be noted as non-compliant on their next inspection.
