In order to quickly identify children at risk for vaccine-preventable diseases in the event of an outbreak, licensed child care centres must submit a current enrollment list annually or as requested by Public Health.
If you haven’t received a username, request one here.
The following information must be included in each list:
- Full name
- Date of birth
- Sex
- Address
- Phone number
Licensed child care centres that do not submit updated enrollment lists will be noted as non-compliant on their next inspection.
