Enrollment List Submissions for Child Care

In order to quickly identify children at risk for vaccine-preventable diseases in the event of an outbreak, licensed child care centres must submit a current enrollment list annually or as requested by Public Health. Your can request a username to access the system.

Submit child care enrollment list electronically

The following information must be included in each list:

  • Full name
  • Date of birth
  • Sex
  • Address
  • Phone number

​Licensed child care centres that do not submit updated enrollment lists will be noted as non-compliant on their next inspection.

If submitting enrollment lists via mail, please address the forms to the attention of WDGPH Vaccine Records.
