Submit out-of-province COVID-19 vaccination

As of September 22, 2021, the Province of Ontario’s proof of vaccination program requires patrons who wish to access certain businesses and settings to show proof of identification AND proof of being fully vaccinated against COVID-19. Wellington-Dufferin-Guelph Public Health is encouraging anyone who received a COVID-19 vaccine outside of the province to take the necessary steps to ensure they are considered fully vaccinated in Ontario.

If you received a Health Canada approved COVID-19 vaccine outside of Ontario

If you are a resident of WDG and received a Health Canada-approved vaccine outside of Ontario (one or both doses), you can submit your proof of vaccination by completing this form. Health Canada approved vaccines include (please note the new names of the approved vaccines are also included below):

  • Moderna Spikevax
  • Pfizer-BioNTech Comirnaty
  • AstraZeneca Vaxzevria
  • Janssen (Johnson & Johnson)

If you require another dose to complete your series, you can book an appointment or come to a drop-in clinic once you have completed the form. Please also bring your proof of vaccination from outside of Ontario with you to your appointment.

If you received a non-Health Canada-approved COVID-19 vaccine

If you are a resident of WDG and received a non-Health Canada-approved vaccine (any vaccine other than the four mentioned above), you can submit your proof of vaccination by completing this form.

In some cases, the Province of Ontario now requires an additional dose of an approved mRNA vaccine for individuals who received a non-Health Canada-approved vaccine to be considered fully vaccinated. Please contact us at ext. 7006 if you have questions about your vaccination status.
