Notice of Intent to Operate a Food Premises Online Form

Under the Health Protection and Promotion Act this form must be submitted to WDG Public Health unit at least 14 days before opening, closing or relocating a food business. This notification allows public health inspectors to review safety plans, ensure compliance with food regulations and arrange for an initial inspection.
