Recreational Water Facility Opening Notification Form

If you’re opening or reopening a recreational water facility in Wellington, Dufferin or Guelph, Ontario regulation 565 under the Health Protection and Promotion Act requires you to notify WDG Public Health at least 14 days before opening/reopening. Please complete the form below.
