POLICY STATEMENT

It is the policy of Wellington-Dufferin-Guelph Public Health that the Receptionist or Program Assistant will deal with routine sample submission inquiries and reception of water samples for bacterial analysis.

PROCEDURE

  1. Collect bacterial samples from front display/counter, review forms for completeness.  The forms must have the following information:
  • Name, full mailing address and phone number of submitter;
  • The location of the water source (well source only, not surface waters);
  • Public Health number (2266); and
  • The date the water sample was collected (sample must be received by the Public Health Laboratory no more than 48 hours from the time the sample was collected.

The laboratory will not complete a water analysis unless the form is fully completed.  Water samples that do not come in the laboratory issued bottles will not be accepted.  Once the form is reviewed, refrigerate the water sample as soon as possible.

  1. Reception should direct public inquiries to the water display for bacterial sampling bottles and pamphlets. Any additional inquiries can be directed to a Public Health Inspector (PHI).
  2. For non-routine chemical sampling requests, direct clients to speak with a PHI.
